Readmission of a patient for preeclampsia complicated by a postdural puncture headache, back pain and PRESS: a case report

A 29 year old woman who had delivered at an outside hospital seven days prior, was admitted for signs and symptoms of preeclampsia and a severe headache. She was placed on a magnesium sulfate infusion. During her labor, she had an epidural placed with some difficulty, and she had residual localized back pain. Her headache symptoms were consistent with those of a postdural puncture headache. A blood patch was recommended, performed with ease and headache relief began before the procedure was completed. After the procedure, the patient complained of increased back pain and pressure. Hours later, the patient’s headache returned, the back pain remained and she complained of worsening bilateral leg weakness. MRI of the lumbar spine and brain were performed. MRI of the lumbar spine was normal, while that of her brain had features consistent with PRESS. Leg weakness resolved when the magnesium infusion was discontinued and the patient was discharged home with fiorcet for her headache. The implications of symptoms with multiple possible diagnoses in this postpartum patient will be discussed.
